
The Arc of Multnomah -
Clackamas’s Family Case Coordination program provides families
ongoing assistance with life management skills in the following
areas:
- Finding and applying for housing, including subsidized housing programs
- Advocating in child
custody processes
- Accessing entitlements such as SSI, TANF, Oregon Health Plan and food
stamps
- Participating in
school (IEP) meetings
- Attending medical appointments
- Completing paperwork
- Providing information and making referrals to other agencies
- Providing limited financial assistance
Eligibility
- Must have with a diagnosis of mental retardation or other developmental disability
- Must be a
member of a biological or adoptive family where school age children are present or involved. The
individual with MR/DD can be the child(ren) or the parent(s).
- Cannot
have community services that appropriately meet individual needs caused by the disability
- Must
be eligible for Arc services
